About Form B – Application for RA Scheme

  • Purpose: Form B is the main application form for seeking compassionate appointment under the Odisha Civil Services (Rehabilitation Assistance) Rules, 2026. It is to be filled by the eligible family member of a deceased Government servant.
  • Who Can Apply: The application can be submitted by the spouse (if alive, below 50 years, and medically fit) OR by the nominated family member (if spouse is ≥50, medically unfit, or not alive). Only one eligible family member can apply.
  • Part I – Deceased Employee Details: Requires details of the deceased Government servant – name, designation, office/department, employment status, total length of service, date of death, and complete list of family members with their age, qualification, marital status, and relationship.
  • Part II – Applicant Details: Requires details of the applicant – name, Aadhaar number, gender, relationship with deceased, date of birth, educational qualification, category (SC/ST/SEBC/Ex-Serviceman/PWD/Sportsperson), mobile number, present address, and permanent address.
  • Declaration: The applicant must declare that all information furnished is true and correct. If any information is found false or incorrect, the appointment shall be terminated unilaterally without any notice.
  • Mandatory Documents: Death Certificate of deceased, Legal Heir Certificate (Tahasildar), Proof of age of spouse (if applicable), Death Certificate of spouse (if applicable), Medical Unfitness Certificate (Form‑C, if applicable), Nomination Form‑A (if applicable), Aadhaar Card of applicant, Educational qualification certificates, Registered deed of adoption (if applicable), Death certificate of husband (for widowed daughter/daughter‑in‑law).

Key Instructions for Filling Form B

  • Application Timeline: Form B must be submitted within one year from the date of death of the Government servant. Delay may be condoned by the concerned Administrative Department with sufficient justification.
  • Late Submission: If the application is submitted after one year, the applicant must furnish sufficient justification along with supporting documents. The Appointing Authority shall examine the case and recommend for condonation of delay to the concerned Administrative Department.
  • No Fresh Application: Applications submitted after two years from the date of death shall not be entertained under any circumstances.
  • Minor Child Provision: If the spouse is ≥50/unfit/unwilling and there is a minor child of 16 years or above, the child can apply immediately after attaining 18 years of age.
  • Educational Qualification: The applicant must satisfy the educational qualification prescribed for the Group‑D post. Exception: The widow of the deceased is not required to satisfy the educational qualification if the Appointing Authority is satisfied that the duties can be performed satisfactorily without it.
  • Verification: All applications shall be considered strictly in order of date of death of the deceased Government servant. Where two or more applications relate to the same date of death, the older applicant shall be accorded priority.

All eligible applications are considered strictly in order of the date of death of the deceased Government servant. Where two or more applications relate to the same date of death, the older applicant shall be accorded priority.
What happens if the information in Form B is found to be false?
Suppression of correct information or furnishing of false information in the application shall render the applicant liable for removal from service in addition to other legal action. This will also debar other members of the family from getting appointment under the scheme.
What is the difference between Form A and Form B?
Form A is the Nomination Form – used when the spouse is ≥50, medically unfit, or not alive, to nominate another eligible family member. Form B is the Application Form – the main application submitted by the eligible family member seeking compassionate appointment. Form B must be accompanied by Form A if nomination is involved.
