About Replacing a Damaged Driving License
If your driving license is damaged, torn, mutilated, or unreadable, you need to apply for a replacement. A damaged DL is not valid for identification purposes and can cause legal issues.

What is Form 1?
Form 1 is a self-declaration form for physical fitness. It is required to be filled during the DL replacement process. It declares that the applicant is medically fit to drive.
- Form 1 is a self-declaration – no medical certificate is required (unlike Form 1A for applicants above 40 years).
- It is an online form that needs to be filled during the application process.
- For applicants above 40 years, Form 1A (Medical Certificate) is mandatory instead of Form 1.

Follow these steps to replace your damaged driving license online through the Parivahan portal:
Step 1: Visit the Parivahan Portal
Open your web browser and go to the official Parivahan website:
Click on Online Services
On the Parivahan homepage, click on "Online Services" from the main menu.
Select Driving License Related Services
Click on "Driving License Related Services" from the dropdown options.
Select State – Odisha
From the state selection menu, choose "Odisha".
Click on Driving License Menu
Click on the "Driving License" menu option.
Click on Services on DL
Click on "Services on DL" to proceed.

Enter Driving License Number & Date of Birth
