About LTC Application Form

Purpose: This is the official Leave Travel Concession (LTC) Application Form for Odisha Government employees. It is used to claim reimbursement for travel expenses incurred during LTC block years.
Eligibility: All permanent and temporary Government employees who have completed the required service period are eligible to avail LTC as per the Odisha LTC Rules.
Block Year: LTC is applicable for a block of 4 years. The current block is 2026-2029. Employees can claim LTC for travel undertaken during this period.
Who Can Travel: The employee, spouse, children, and dependent parents/siblings as per the definition of "family" under LTC rules.
Entitlement: Employees are entitled to two LTCs in a block of 4 years. One LTC can be availed for travel within India (Home Town) and the other for Anywhere in India.
Mode of Travel: Travel can be by air (for eligible officers), train (1st AC/2nd AC/3rd AC), or road. Reimbursement is limited to the fare of the entitled class.

Instructions for Filling LTC Application

Block Year: Mention the correct LTC block year (e.g., 2026-2029) for which the claim is being made.
Destination Details: Clearly specify the place of visit, date of departure, and date of arrival.
Travel Mode: Select the mode of travel (Air / Train / Road) and provide ticket details (PNR, flight number, etc.).
Family Details: List all family members accompanying the employee, with their relationship and age.
Declaration: The employee must declare that the travel was undertaken as per rules and no other reimbursement has been claimed for the same journey.
Required Documents: Attach original tickets (or e-tickets), boarding passes, hotel bills (if applicable), and certificate of no claim from other sources.
Submission: The duly filled form along with all supporting documents should be submitted to the Drawing and Disbursing Officer (DDO) for approval and processing.
